Short answer
Beholder 3’s June 1, 2023 Update 1.1.1 explicitly fixed disappearing quest steps in The Reform. Check for that update before retrying the quest; an older save may still retain a separate problem.
Scope: Steam PC patch record. The checked notes do not establish identical console patch numbers or automatic repair of every old save.

The missing objective had a specific follow-up
Harhaluulo’s 2022 Steam post describes asking people in multiple rooms about The Reform. The quest then disappeared from the list, the phone report could not be made, and a game over followed later. That is the player’s observed sequence; the report does not establish the underlying code cause.
What the player tried before the fix
The same poster reported a different result after loading an earlier save.
- Return to an earlier point before questioning the rooms.
- Go directly to the person responsible.
- Complete The Reform through that conversation.
A later School Problems failure remained in the account. Completing The Reform did not prove that the separate task was repaired.
The official update changed the answer
Update 1.1.1 lists disappearing steps in The Reform among its fixes. The preceding Major Update 1.1, dated May 24, 2023, also recommended a new game because some corrected problems could persist in old saves. That older-save warning is relevant context, not proof that every existing campaign must be discarded.
What to do with a campaign now
Install the available update and retain a copy of the campaign before testing the affected objective. If the problem continues, report the platform, build and the last completed step. Do not turn the historical workaround into a requirement to avoid speaking to every NPC in every current version. The concrete risk in the original report was losing the route to the phone report and having to replay earlier progress.
